Abstract:

The evaluation forms are designed to gauge the effectiveness of the community health workers (CHWs) according to their supervisors. Community health workers’ supervisors completed and submitted forms quarterly to the University of South Alabama’s Coastal Resource and Resiliency Center (CRRC) beginning July 2016 through January 2017. CRRC gathered, input, analyzed, and reported data provided by the CHWs to evaluate the utility of these CHWs in building community resilience.

Purpose:

To monitor and evaluate the effectiveness of community health worker resilience-building efforts in the target areas of Alabama and Louisiana.

Methods:

The quarterly activity and evaluation form were administered quarterly and submitted electronically to the Coastal Resource and Resiliency Center by the community health workers’ supervisors.
